Hi,
I am trying to modify a contact form but I do not know why the Dropbox has been duplicated?!
I think the issue comes from script.js but I do not know how to fix it?!
Can you please take a look at the site:

as you can see there are two select drop box while I have created only one in demo.php
the other issue happens in Google chrome (Please open the link with chrome)but not in firefox and IE and is overlapping the textarea name="message" on the input type="text".

Can you please take a look at the link and let me know what I am doing wrong?
