question regarding adding a live search to a form

Hey guys, we have a grad survey thing we are making and the questions are currently in a form. Within the form we have the regular drop down boxes for the users retrieved from a mysql database....

Can anyone point me in the right direction so that we can change those drop down boxes with live search boxes?
