Res. Form/Dreamweaver 8/Email to Address

I'm new to Dreamweaver 8. I would like help on how the following
A. Designing a Reservation Form HTML
B. Submitting that filled form to an Email address by clicking a submit button
C. The simplest and most efficient way to do it.

You need to check your web hosting company for their form mail program. It should have instructions for setting up your form.
