Seem like you have a Smitfraud type infection , download something called SmitRem and run it in safe mode , just to make sure it get rid of everything , make sure you back up all of your data too.

Before removing any thing you need to back up you personal data just in case something should go wrong .

First you need to go to Start - Run - MSCONFIG -startup and untick

C:\WINDOWS\12.exe ...
