I think I figured it out and it's remarkably simple (almost too simple).

I have two ways of doing it though:

This way seems to perform about 10% faster according to my benchmarks.

Thanks. Still though the table would be too large. If 50,000 customers have custom prices for 30 -100 items each, the table would be millions of rows long.
