I need someone to prepare a web-based application that would allow registered members to encode their profile details, picture, biometric (index finger) and personal data, service certificates and clearances. The information will be checked by personnel from an authorized agency using existing hard copies of documents in the agency's position. The usual database functions to register, login, input, edit, update, copy, delete and print must be included together with the permission rules in applying these to the database.

This application is for demo purposes to introduce the concept of SAAS to the agency and must not take more than one day to accomplish.
