Emails getting hacked during the transfer from one account to another

i have a question for everyone, i don't know if this is the right forum to be in.....
when an email is sent from one domain to another (one company to another), how easy is it for a hacker to access that email while its in route....and what type of security would have to be setup to secure that email? don't they have to find a none secure port the email is traveling through (i guess port 25, smtp) to gain access? How would that work?
