Just plain PHP?
I know some awesome PHP / MySQL ones?
http://www.w3schools.com/php/default.asp
http://uk.php.net/tut.php
http://www.tizag.com/phpT/
http://www.freewebmasterhelp.com/tutorials/php
http://www.php-mysql-tutorial.com/
Bottom ones are best IMHO

I just edited my post. check out the attacthed zip. Its a relatively rudimentary PHP/MySQL adressbook. Even if you dont have MYSQL set up you can still examine the code to see how it works
