I haven't been able to figure out the hosts file you're speaking of -- every time I run a cleaning run, I get the long log.

Any other suggestions?

Surprise - the blue desktop is back! I have no idea how, why or what did it, but I'm cautiously optimistic.

Is there anything else that's worth checking before we declare this solved?

We-ell... if it's all working SMF probably cleaned the adware out.
The hosts file is a redirection file for URLs that exist in the listing.... in your case if you type any of those URLs into your browser it is redirected back to your machine [127.0.0.1]; that file would have been written in there by one of your spyware guard services.
So yep, you could be okay...[I don't actually like loading the first page of this thread...]
Be safe.
Deep, deep in the woods, but walking about.
