You will need a static IP address for your home pc. The remote pc will connect to your home PC using that address. You can connect remotely by using Real VNC or Remote Desktop Connection (included with Windows).

You need to setup 'Remote Desktop Web connection' if you want to Access via net. See more information in Windows Help.
