Desktop Icons Messed Up After Every Sys Restart

Every time I restart my computer, the icons on the desktop go into columns, although I set them up in a particular order. Also, the quick launch toolbar gets disabled. Is there any way to prevent this from happening and what do you think is causing the problem?

After you get them the way you want them, are you then locking your desktop?

Also, the things you gave me are for restoring the layout. What I don't understand is why it's getting messed in the first place every restart.
