Program Question...listboxes

Ok I'm trying to create a program here and i have three buttons.

Add
Clear
Exit

when i enter the name/title and click add i get the name/title in the listbox but as soon as try to add a new name/title it just replaces the old one. could someone help me out with this code? Thanks, Scott
