first of all u did not post any error
second in code please include ur line breaking tags as text
i.e.
vbcrlf & "Pax5 :" & request.Form("pax5") & vbcrlf & "DOB5 :" & request.Form("dob5") & "<br />"

if not solved problem then please paste error message
