Newbie needs help with billing form

I need some sort of billing form for my web site but i don't now where to start. Only got some knowledge of html and average knowlege of dreamweaver MX 04.

Try www.hotscripts.com, they have a good 100+ of different billing scripts on different platforms (ASP, ASP.net, PHP etc...)
