How to create multiple toggle field codes in Word.

I am using the toggle field codes option to populate a report from a Word document. I have not been able to get this option to do multiple parts of a scenerio (ex. Does this condition exist? If yes, than do this. If no, do that.) If more information is needed please ask (document is attached with example).
