upload and download file through forms

I need to add in my forms ability to upload and download files in any extension (or if it is not possible it can be PDF only)

i build upload .jsp and .html to upload so i have file name and path of it in table

So my question is to find way to download these file?

and if it possble to make upload file in forms page only without jsp and html
please help me
