displays on top of the page

i need to include this file in some other php file if i add a plain text where i want it to display it works but when i include this file it just displays the form and the comments on top of the page rather then where i want it. would you please help me.

even if you please could point me to right direction would more good enough. i took of line i mess the code in a lot ways but it always displays on top.
