Drupal - How to link a filled out form already to a feedback form?

Can I link a filled out form, to a new form?

I want one user to fill a form with certain characteristics - for example, a patient.

Then, I want another user to take this particular form (select it from a table) and then perhaps create another form - based on certain characteristics in this form - let's say patient number, ID, first name, last name, and case #.

i.e. something like one hospital creates a patient encounter, and the next hospital provides feedback on that particular encounter - a referral to a specialist, and a specialist replying to that result.

What is the best way to do in Drupal?
