Check your inbox for my customizable solution - uses arrays so that you don't have to rewrite the logic every time you change a form field or scaling factor. I've simplified it so that it uses 4 X 1-D arrays as opposed to 2 x 2-D arrays. If anybody wants me to post it here I will, but I assume this is a dead thread by now.
