Do you know the easiest way implement (3 drop down menus + 1 picker)

I want like [3 drop-down menus / picker] at:
http://www.russian4foreigners.com/node/298 (end of page)
implement mine at:
http://www.poliscarhire.com (left side of page)
currently exist ONLY 3 drop down menus...
I think above site used Drupal & jQuery (source code)... well, I want as you see in my site, a total of 2 components each of (3 drop down menus + 1 picker)...

Do you know the easiest way implement this?
