I have a form that I want to put on the web for online form submission, below is how it ought to work in my mind. I need to know whether this can be a reality and how to do it:

Create form in Word.
Convert Word form to PDF.
Enable PDF form for online submission.
Collect results from submitted forms in Excel document.

Is this possible, and how?

Create form in Word? Yes.

Convert Word form to PDF, yes. Office 2003 onwards allows you to do this by either 'Saved' as' or exported. One or the other.

Enable PDF Form for online submission? Yes, if you have access to Adobe Acrobat Pro. (not Reader) There is also a little bit of a leaning curve to this.

Collect results from submitted forms in Excel document? Yes.
