I want to create a grey login bar at the top of my page but I don't know how to make one please could someone help.

The code that I want in the login bar is

I want it to be like this username:<FORM> Password:<form> Login Button.

how about having it in one tr only <tr>name<form>pass<form><button><tr>
then if you want it to be on the top then have these property on the table padding-top:0;margin-top:0;
