help on file missing in the loop

i've generated dynamic pdf files from php script with mysql
which suppose to have 25000 files. The problem is after the loop i only have 24978 files and 22 files is missing. I couldn't find the problem and which record did not process.
i was thinking of the solution reverse the flow which read the files from the folder and record it into the database and not manage to.

any help is needed urgently.

my big careless unforgiven mistake to myself everything is okey but didn't realize the record contain "/" symbol that is why the file is not saving.
