I am trying to use the pc as a recognition service
I want to take the calls (the number of the caller) and use it
to an html form
I really need the part
that i take the phone number of the caller and pass it to a form then
i know how to connectthe database and all
i don't mind if the passing is done through javascript or vbsript or even xml
i use localhost and connecting to vox is not an option

Yes, you can check out this Reverse Cell Phone Number Search site. It provides free information such as carrier, location (city and state) and phone type (cell phone or home phone). This place doesnt actually take calls, they give you a phone number to use and when someone calls it they forward the call to whatever phone number you want them to.
