Helo everyone,

I have installed a php directory script similar to this at:
[http://www.dentalarticles.com/directory/add_url.php?c=303]

and I wonder if someone could possible tell me how to add 3 additional fields to the submission form.

Is this something that could be done by someone with almost zero programming knowledge? !!!
