I need a script and am not quite sure of the best way to write it. Any help would be greatly appreciated.

Basicly I need a form that collects the following data User name, User id(number) module name, module id(combination of letters & numbers), the year and i need a place for comments to go.

This data needs to be stored somewhere once submitteda message should say that its been saved.

I also need to be able to retrive this data and display it in an html format on a sepearte html page.

Im useless at this side of things so any help would be hugely appreciated. Or poiting me to sites or scripts that are similer would be amazing as well.

Do a Google search for "PHP formmail script". You'll find plenty of existing scripts to use.
